The post will be to support reablement in Adult Social Care so will be familiar with an enabling model of short term intervention, completing assessment (be familiar with Best Interest decision making), commissioning support and working as a multi-disciplinary team member in both hospital and community setting. This post will preferably by 37.5hours.
SOCIAL WORKER (ADULT SERVICES)
To work as part of a multi-disciplinary team to enable individuals achieve their outcomes from a strength based perspective which promotes co-production for individuals, carers, families and communities, while fulfilling statutory responsibilities and applying values, ethics, knowledge and skills appropriate to Social Work Practice.
Professional qualification in Social Work and must be registered with Social Care Wales. DBS and references required.
The candidate must have up-to-date experience of working within an adult social care social work team, with an active knowledge of the Social Services & Well-being (Wales) Act 2014 and its application to assessment, with a working understanding of eligibility, best interest decision making, local knowledge to offer sound information and advice and an overview of safeguarding procedures.
The successful candidate will have knowledge and experience of working within a multi-disciplinary environment and the motivation to work as part of a team, which may involve taking the lead in care planning and actively contributing to the arrangements of complex care for adults over the age of 18.
